1. Complete the Helping Hands & Hearts Client Services or Caregiver Application
   If you are an individual with cancer, complete our Client Services Application
   If you are a caregiver of an individual with cancer, complete our Caregiver Application
   
   
2. Submit the completed application to Helping Hands & Hearts Foundation either in person to a Board member or mail to:

3. The application will be reviewed and you will be contacted so we can better understand your needs and how we can help, typically within 7 – 14 days.
